Description

This session equips nurses and healthcare professionals with the communication skills needed to facilitate meaningful, values-centered advance care planning (ACP) conversations with patients and families.

Grounded in the understanding that ACP serves as a blueprint for living, not simply a document to be signed,  this session prepares clinicians to move beyond checklist-style questioning toward authentic, patient-centered dialogue that honors what matters most to those they serve. Participants will learn to use a structured communication framework to guide these discussions, distinguish between a patient's stated preferences and the deeper values and goals driving them, and critically evaluate whether a conversation has truly captured a patient's wishes within their cultural, spiritual, and family context.

Learning Objectives

By the end of this session, the learner will be able to:

1. Differentiate between a patient's stated treatment preferences and their underlying values, beliefs, and goals of care when conducting an advance care planning discussion.
2. Evaluate the effectiveness of a completed advance care planning conversation including its responsiveness to the patient's cultural, spiritual, and family context - in accurately capturing and honoring the patient's end-of-life wishes.
