Starting Compassionate Conversations: Advance Care Planning

Description

This session equips nurses and healthcare professionals with the communication skills needed to facilitate meaningful, values-centered advance care planning (ACP) conversations with patients and families.

Grounded in the understanding that ACP serves as a blueprint for living, not simply a document to be signed,  this session prepares clinicians to move beyond checklist-style questioning toward authentic, patient-centered dialogue that honors what matters most to those they serve. Participants will learn to use a structured communication framework to guide these discussions, distinguish between a patient's stated preferences and the deeper values and goals driving them, and critically evaluate whether a conversation has truly captured a patient's wishes within their cultural, spiritual, and family context.

Learning Objectives

By the end of this session, the learner will be able to:

  1. Differentiate between a patient's stated treatment preferences and their underlying values, beliefs, and goals of care when conducting an advance care planning discussion.
  2. Evaluate the effectiveness of a completed advance care planning conversation including its responsiveness to the patient's cultural, spiritual, and family context - in accurately capturing and honoring the patient's end-of-life wishes.

Ottamissiah (Missy) Moore is a Registered Nurse, End of Life Doula and a resident of Washington D.C. Her wealth of experience and knowledge in nursing spans over 38 years. Missy has extensive experience in Long Term Care Support and services, Wound Care, Hospice, and Dementia Care.  She provides consulting services for conference planning and trainings to include leadership, hospice and wound and dementia care.  Ms. Moore provides support and education to patients, caregivers, faith communities and professionals on health-related issues.  Ms. Moore is currently a DNP student at Kentucky State University.
